On a recent job, we tried a new approach to installing custom-milled baseboard over a concrete slab. We had the base milled 1 in. wider than usual, and we asked the supplier to mill a casing profile on both edges. Another shop stained and varnished the material. When it arrived on our job site, we ripped an edge off the base and installed it as base shoe, as shown in the drawing. It matched perfectly in color and grain.
