The size, both length and diameter. Brads are usually 18 guage, and about 1/2" to 1-1/2" long. Some guns shoot up to 2" brads. There are also 23 ga nailers available, but they are used mostly by furniture makers who need a very, very small brad.
Finish nailers come in 16 and 15 ga. You could get by with either, but the 15 tends to have longer nails, and is more common.
Brads are generally used for attaching small trim, like quarter round and some of the smaller door trim. Finish naileerrs are used for hanging doors, crown moulding, etc.
