There was a book about old door and sash making that was published by Bicycle Press some time ago. My copy is packed away somewhere, as I am in the process of moving.
Two important thoughts. One; wood will move! You have to build the door allowing for the seasonal movement of the wood. Two; use heavy hardware. A typical plank door will use planks of 2" or more, add to that the Z braIing (or whatever you like) and you have a HEAVY door.
You might try searching the wood working forum, I'll bet there are some threads about this over there.
