Task: I need to cut the hole in the floor for the rough-in toilet plumbing.
Question: Are toilets standard such that I can cut the hole and all toilets will fit?
Question: if so, what size hole should I cut (3 or 4 “) and how far from the finished wall should the CENTER of the hole be?

Why even bother thinking about it so much? Buy the toilet and measure. But the 3 or 4 inch hole will be dictated by the size of the pipe coming up through the floor.

Toilet drains are located 12 inches from the CENTER of the drain to the finished wall. Yes ... toilets are standard such that you can cut the hole and all toilets will fit.
The water supply line is traditionally located 6 inches high from the finishes floor and 6 inches to the left of the center line of the toilet drain.
The drain line will be 3" pipe ... running up to meet a 4" X 3" floor flange... 3" hole.
This is an easy, fun job ... you'll enjoy it.
I've built in a few toilets from scratch ... something 'deeply satisfying' about that eventual first successful flushshshshshshhhhhhh.....

You're 'jaded' to the experience!
I'm a lone, one thing at a time, non-pro builder. For me, to suddendly have a working bathroom where there always was none .. is still sorta amazing. Some tiny little somethingorother in me is always wondering, right from the start on through to the finish, whether or not it's all 'really gonna work' in the end. So for me ... yeah ... when it's all plumbed out beautifully and the shower, sink and toilet are in and it ALL WORKS! ... call me naive or whatever .. but that sorta thing still brings me great delight! Proly wouldn't be true if I did "two or three a month". But right now I'm glad for it. That sort of joy in the work -- silly as it may sound -- is still really fun for me.

I think 12" in back and 18" on the sides is typical.
Check the framing below before you start cutting. You might be able to shift it a hair and avoid problems.
I can resist anything but temptation.
Minimum side wall to center is 15" although there is a little room to cheat in tight situations.
From the back wall it is "usally" 12' but can vary with some of the newer toilets.
