I'm not aware of any books dealing specifically with this subject but you may want to check the archives here at Breaktime and at The Journal of Light Construction. You also might want to try the online book catalogues at John Wiley and Son and McGraw- Hill or The Craftsman Book Company .....they may have something. Post tensioned slabs and slabs on bell bottom piers were fairly common in the Gulf Coast area ........both seemed to work well if done right. Good Luck
