My two year old Bosch hammer drill has a bad habit of the chuck loosening up after a few seconds of drilling in concrete. Doesn’t matter if the drill bit is brand new or used. Drilling 5/16, 3/8, and 1/2 holes the chuck loosens up on all of them.
What’s my remedy???

I try to keep my hand(lightly) on the chuck when it's turning.

Do you tighten all three holes in the chuck? The proper way to tighten a chuck is to snug down each of the three holes to align the bit in the jaws and then go back and torque each jaw firmly to actually hold the bit. If the bit is not aligned the jaws don't grip the shank of the bit and the chuck can loosen and the bit gets scored. Once the bit is scored it is very hard to get it to hold in the chuck since the bearing surface of the jaw/bit interface is minimal. Do it enough and the jaws of the chuck become worn.
In normal drilling this is not critical but still good practice. For heavy duty work it is very important.
