Check this video out to find out about pouring a thin slab over subfloor to deaden the sound and also make a fabulous radiant heat thermal mass floor. (must be engineered to carry the weight etc)

You need to add an isolation membrane between the subfloor and finish floor.

Membrane you suggest = $2.30 / sq ft
Homasote = $.84 / sq ft
Extra layer of 5/8 drywall = $.30 / sq ft
Double up your drywall or add a layer of Homesote Board under the drywall. Simple trumps complicated every time.
No, not the same thing at all. None of those will do the same thing as a thick sound membrane.
I like to instal a drop ceiling below and fill every last hole in which sound loves to travel
When building a frame house, the cavity of the walls is filled with insulation, however, for good sound insulation, it is not enough. For noise reduction, a “pie” of drywall and mineral wool is installed on the inside of the wall. Fastened "pie" with metal brackets to the wall or screws. Such a connection should also be isolated from noise using rubber gaskets or use a ready-made mounting system, including the presence of vibration-absorbing gaskets.
