I would like to install a kitchen in my walk-out basement with a grilling station so I can grill all year long. Is that permissible?

Probably need to check with the permit office. Sometimes if you put in a second kithen they think you qualify for two-family status, which may not be allowed.

My initial thought was to just buy an upscale gas grill (propane) with a granite countertop, etc. and install a very good vent system. I could convert the grill to natural gas. Any worry about smoke issues or CO? Do modern gas grills burn as efficiently as gas stoves or gas cooking tops?

What you're talking about is technically a "stove" or "cooktop". There may be regulations limiting the number of kitchens, as suggested (though you may be able to squeek through even then by calling this a "bar" or some such). Whatever cooktop you use must be designed to be used indoors. And there will generally be code issues requiring some type of hood or ventillation device.

Depends on the details. Might be as simple as having an exhaust fan. Might be "no way, no how." That can depend on your AHJ (Authority Having Jusridiction), and what they say.
Do you mean that you want an equivalent to the outdoor grill I'm guessing is outside the w/o/b door? That can be different than a gas-fired grill unit for a counter-top cooktop.
Or, maybe, are you thinking of one of the outdoor cooktops under some sort of cover adjacent to the outside of your basement? Then, it's likely down to your climate (which "we" here could know about, if you'd either click on your handle in blue, or the "Update Profile" there in the brown band at the top right of the screen).
I had one of these Jenn-Airs in my house with the gas grill built into the top http://jennair.com/catalog/category.jsp?cat=69&N=56 (the other side is two gas burners). Nice concept but the down draft was so strong it pulled the flame/heat away from the grille area.
There's lots of commercial grill's available for interior cooking but you need a commercial grade exhaust system to match...they both equal $$$.

Here is one I put in. Its about 40k btu and the hood has a 1200cfm fan..Use it all year .. Hope pic is not to big...maybe someone could shrink it if so...

I have my natural gas grille hooked up 2 feet from the back door. I cook on it 24/7 x 365 if I want. Love it. It sits on my treated wood deck and backs up to a small privacy fence so I don't have to look at the neighbor while I cook.
Thanks for all the great input. I guess I will install a conventional cooktop in the kitchen and a gas grill just outside. No problem with the AHJ thinking it is a 2 family home. A basement rec room with a 2nd kitchen is just fine according to the BI (Building Inspector).
