I’m working on a half bath gut job in my own home and doing all the work myself. Needing to rerout the water supply lines I took up the floor. While I’m at this point I figured I might as well address the WC drain. What I have is what I think is the old lead and oakum type connection to the cast iron pipe. The top of the lead pipe is pretty chewed up. Although it is maleable I’m wondering if I’d be better off removing the lead and adding PVC. One reason for doing this would be that it would be much easier to make the flange connection rather than basically doing a custon fit job with the old pipe. This would also allow me to get the flange at the proper atop the finish floor height. My question is regarding a connection to the CI. It appears that this is a 4″ ID pipe that has a shallow hub. Would one of those rubber transition clamps work for a situation such as this? I’ve seen them at HD and believe there is one that appears to be a step down type. If this is the case would you simply slide 4″ PVC inside the CI down to the bottom of the hub? I hope my question makes sense. Any help would be appreciated.

If you can get access to the pipe further down stream then I would cut off the flange and use a rubber transition coupler as you decribed to go to PVC or ABS.
The cutter can be had at most rental tool places. Looks like a big bicycle chain with pipe cutting wheels. This will give you the straight and clean cut needed for a good transition to plastic.
To lubricate the rubber sleeve for easier installation, I've used silicone sealant as a lub and extra sealing power on the Cast iron. especially on a horizontal run.

By the flange, do you mean the hub of the CI that has an OD of about 5.5"? Thanks
No I mean beyond the hub that you are talking about. The OD should be about 4 1/2 ". Cut off the flange that allows you to insert another CI pipe.
The differance between a ci pipe and a pvc pipe should be about 1/4".
