I need to mount 4″ square x 1 1/2″ outlet boxes to the concrete forms for outlets in an inside wall. They will be connected to each other by 1/2″ emt. I am concerned that if the boxes are not secured strongly to the form board it will be dislodged during the pour. Any ideas?

Using screws, attatch a block of wood the same size as your box to the forms. After the formboards are stripped, set your box with, perhaps, concrete screws.
For minimal damage to the slab when stripping the forms, use a different material for the block. Maybe foam, or layers of cardboard(that really thick stuff).
I'd suggest you be at the jobsite the day of the pour...lucky you. I mean, they certainly woudn't care about the placement of your boxes, right? Just, no fighting, you hear me?

First thing to do is check your local codes, where I live, the city code is based on the NEC (which allows EMT in concrete, or at least the old edition I have does) but the city has amendments which, among other things, prohibit EMT embeded in concrete.
Seems to me the soultion is to just secure the boxes strongly :) I think I'd try using tie wire through nail holes in the forms, even if person doing the stripping forgets to cut the wire, it will probably break before destrying the box. Even though is sounds like Rich is talking about instaling the boxes after the pour (how does one attatch the conduit?) the idea a using a block of that would fit inside the box, nailed to the form to position the box (before/during the pour) sounds good, or one could just use a couple nails at the inside corners of the box (inside the box so they come off with the forms and are not embedded in the concrete)

Normally, the box assemblies c/w conduit is nailed to the inside face of the forms.
I don't like the use of screws or any fasteners that destroy the surface of the forms.
A little care is all that is required during the pour.
This way when you remove the forms, everything is where it should be and the forms can be re-used until they pay for themselves.

You'll do yourself a big favor by using 3/4" conduit instead of 1/2'. It's only a little bit more expensive, should still easily fit in the wall, and with more than twice the cross section area, it makes the wires alot easier to pull. After all, in this application, you won't be able to replace the conduit later;)
