Pretty simple. Read the chart from their wesite. As far as over winter work clothes if you got to ask then you don't need to be around tools.
How to Size Your Tool Belt
Place your existing tool belt in a straight line and measure it from the end of the buckle to the most commonly used hole. Select the belt size (by mid point) that is closest to your measurement.

If you do not have a current tool belt to measure, you can get an approximate tool belt size by adding 4" to your pant waist.

Problem is, I think I am an in-between, wearing 34 Levis, and a medium will snug up fine in summer, and not fit in winter. We have no retailers within a two hour drive where I can try anything on.
Guess if I want Occi I'll have to go Stronghold adjustable.

Gene, I'm a 33/34 jeans size too and I have a medium occi belt. I fell for it too the first time around and figured I "must" need a large. Nope. Bad idea. Order the medium and if it doesn't fit, I'll send you the large that's been in my basement for two years.
Good time of year to buy it too. The leather will stretch some over the summer break-in. You'll drop down at least one hole in the belt by the time it's done breaking in. Come winter time you won't have any problems getting it around your winter bulk without resorting to the dreaded 'last hole'.View Image

Get the large size and then go to a leather or (horse) saddle shop and buy a hole punch. In the summer, pull it down to fit and punch your hole for the belt buckle. Why wouldn't that work or am I missing something here?
Take a look at the earlier post in which the poster has pasted in the "how to size your belt" stuff from the Occi site. See the image of a belt?
The belts are cut with larger width where the bags and tools hang, and a narrow short tongue where the holes are for the buckle. There is a length limit to where one can punch holes for the buckle.
Here is what I ended up getting. Adjustable for waist. Should take care of the "I'm in the middle between medium and large" thing, and can be worn over winter clothing.
