I’m trimming out an addition and this is the first time I’m using plinth blocks. On the side of the doors will be 3 1/2″ flutted moulding, and I’m wanting to use a plinth block at the bottom. My question is, should the plinth block be flush on the side towards the door, or should it equally overhang on both sides ? Plinth blocks will have a 1/4″ cove edge on front 3 edges.

Your fluted board should reveal maybe 3/8" of the jamb and then the plinth could extend an extra 1/4 inch wider than the fluted trim on both sides.
All in all, I think the plinth should be centered with regard to the trim that it is associated with.

All the plinth blocks I've seen (which is maybe 10 houses worth or so) the revel of the jamb is constant. the plinth block is even with the casing on the inside edge, and extends past the outside of the casing about 1/4 to 3/8". Then backbanding (though I doubt flutted casing has backbanding).
Equal reveal
We set casings showing 1/4" reveal on the jamb, and the plinth is about 1/8" to 3/16" larger each side of th ecasing, depending on the overall scheme and casing thickness. Yours is small so I would make the plinth 3-3/4"
But we barely ease the edge of the plinth. The thickness of the plinth and the size of that cove edge will have an effect too. I think you'd probably have to make the plinth at least 1/4" thicker than casing to front and to each side to allow for the cove. That is a lot for such a small casing.
