Has anyone ever seen a pear shaped precast concrete pier with a screw jack impeded in the top. I have seen them advertised in the past for giving additional support to beams in a crawl space but I haven’t been able to locate a place to purchase them. I have found one company that sells them locally but they never seem to have them in stock.

I've never seen a precast pier with an integrated screw jack, but you could use one of these from Simpson, fitted to a standard precast pier:
