Just had a radon removal system added. The vent is a downspout sticking above the roofline. Does this downspout need some kind of cover on it?

It's just a slab vent and yes, all they used is a downspout.
I see a lot of that around here (chicago). I think it looks better than PVC. It matches the downspouts on the rest of the house.

if you can't find a cap to fit use one or two elbows with a piece of wire fabric inside of one to limit the pest

Most systems I see around here have a 4" PVC stack with nothing more than an elbow at the top to keep out most of the rain. Seems like anything that is directly connected to the potable water supply would have something more substantial. If it is just a sub-slab vent system, that's probably sufficient.
Does the system include a power blower?
How do they flash around the downspout? I've never seen anything but a standard plumber's boot with a pvc vent pipe. You could add an elbow to keep the rain out, but I would find that ugly on a roof so I'd go with the hat-style cap.
A gather it is a retrofit. Maybe the downspout was installed on the side of the house... That would make some sense in that he said that it went with the other downspouts on the house. I too had this pic in my mind of a downspout coming up through the roof and was somewhat surprised. Maybe not.
I think you're thinkin too much, see the photo attached. Pretty simple installation. Code doesn't allow the blower in the house, so you see this arrangement all over town.
I also added a pic of my neighbor's house. I didn't want to get too close when I took it. It is 7:00 in the morning and I didn't want to be the guy that walks around taking pictures of my neighbor's pipes.
What's the total cost for something like that?
Before I give the price, it includes...
One pvc pipe into the basement floor, one into the family room floor (we live in a split level), blower, new circuit, sealed sump pump cover and the decorative downspout vent.
All for $1,200.

You say you are outside of Chicago? I never knew Radon was a problem there? Interesting. Did they tell you why?
I'm curious as I live in Florida near the phosphate industry and many folks are concerned about it here but didn't know it was much of an issue elsewhere.
