I’m renovating our bathroom and have a cast iron stack and toilet drain. I replaced the flange and will be installing a Oatey PVC drop in toilet flange.
My tile guy had cut the tile around the flange so it would sit on the sub floor and not the finished surface. I know the flange needs to sit on the finished surface as to prevent any issues upon toilet installation.
My question is, I plan on using a 1/4” flange spacer to accommodate the difference in flooring heights. Should I put the spacer down first, and place the replacement flange on top? Or set the toilet flange, and then add the spacer? Or does it matter?

I would put the spacer under the flange. that way your flange will seal inside the cast iron pipe, and the wax ring will seal between the wc and the flange. placing the spacer between would mean one more surface to seal.
You may want to clean up the inside of that cast iron for a good seal between the drop-in and the pipe. don't want to chase strange smells, (or a leak should the line back up sometime down the road.)
Do you have some solid material under that floor around the pipe to secure the flange into?
There’s the original 3/4” plank sub floor, then I installed 1/2” plywood before laying the Hardie backer board to flatten the surface. So yes, I do have material to screw into
