It is either dead air or white space but I am half blind and half deaf, so I'm not sure.

It's an indication that you need a better camera.
It is a little out of focus.And it would help to have a little more details about where this, age of the house etc.I am thinking some kind of pilot light or night lighht behind the grills.Or maybe an annuciator of some kind. A buzzer or something like that. Maybe a servant call.And it might have some history in quakery such as emmitting a "blue light" to kill germs. Or an early version of the plug in "oder makers"..

I think Piffin is on the right track - some kind of old buzzer / intercom dodad.
Have seen nightlights in hallways with similar type grilles - but if this was a nightlight, you would have already figured that out when the grille fell off in your hand and there was a bulb or at least a socket inside.
JimNever underestimate the value of a sharp pencil or good light.
I don't know. That would be some serious sized old speaker wire.
So we're thinking a small speaker under the large grill and a mic under the small?
