In terms of emergency egress requirements, how is the net clear opening measured for a double hung window? Is it the size of the opening with the lower sash lifted, or is it the size of the opening with both sash removed?

It is the size of the "openning"
So open your window.
Don't dismantle it, just "open" it.
The idea is that in the kind of panic most folks find themselves in with the house burning down behind them, they are lucky to have the presence of mind to figure out how to open it. Most could never manage taking the sashes out in those conditions, heck - I get customers who need me to show them every year how to flip the sashes in for washing.
Check with your local jurisdiction for code requirements. The opening has to be large enough for a fire fighter with an oxygen tank on his/her back to get through.

Local codes may vary, but the nationally adopted ones require 5 square feet of free opening acessible without using a key, etc; sill no higher than 42", and with minimum width of 20" and minimum height of 22". Note that a 20x22 window does NOT have the 5 square feet!
