Can anyone identify my vinyl windows based on the image below ? I need to replace a broken sash and after checking every window in my house this label is the best info I could find that was still “intact”

is the glass broken/damaged in the sash or is the actual sash damaged? my local glass shop replaced the glass in a sash after it got "foggy" - and we had the paperwork for the lifetime warranty for this window from the installer and the manufacturer (both long out of business/"retired")
